Leadership Course Application- return to Ms. Foster BEFORE registering for the course. 

In order to enroll in Leadership, you must complete the following criteria and submit it by the given deadline.  Once all has been completed, you will be notified if you are selected for enrollment in the course. 

1.  Resume (Please type)

a.   Name, address, phone, email, year of graduation

b.      Current membership of organizations within and outside of the school.  List your title and duties within each organization. 

c.       An essay which addresses the following points:

                                                                                      i.      Why you feel you would benefit from a leadership class.

                                                                                    ii.      What you feel you could contribute to the class.

                                                                                  iii.      How you define the role of a leader.

                                                                                  iv.      How you see yourself in your daily life as a leader.

                                                                                    v.      Your leadership role model- who they are and why they are a role model for you as a leader.

2. Recommendation

a.       A brief letter of recommendation from a teacher and/or activity sponsor describing why they believe you would benefit from a class in leadership and should be considered for this course.  He or she should address specific characteristics of your leadership potential.  This person may be any non-related adult who has witnessed your ability to lead.
