If your child needs to take medication during school hours, an "authorization to dispense" form must be completed by the parent. The completed form and the medication should be brought to the office by the parent. Do not send medications to school with your child. Medications need to be clearly marked in the original container or prescription bottle.
Notify the school if you have a change of address. The change of address form along with 2 proofs of residency need to be submitted to Student Services. Administration Building. Bus stops will be updated after the paperwork has been received.
Grades, attendance, and lunch accounts will be available through this link. Please fill out the form that is linked here, and have your student bring the completed form to the school office. You will then be assigned a PIN number and password through e-mail. Any question regarding Parent Connect, should be directed to zangle@gischools.org.
Permanent Bus Change Request Form
Permanent bus stop changes can be requested through the Transportation link on this website. Use this form if you require a permanent change for your child for the school year. Completed forms should be submitted to the school office for approval.
Internet Acceptable Use Policy
Before students are allowed to access the school's network, students and parents must first sign the internet policy use form. This gives students and parents the guidelines of what is expected of students when accessing the world wide web.
Kroger Rewards Reenrollment Form
Thru the Kroger Community Rewards Program you may donate to your child's Washington D.C. account to help offset their 8th grade trip by using your Kroger Plus Card. Everytime you used your Kroger Plus Card a percentage of what you spend will be send to the middle school, which will them be put into an account for your child's WashingtonmD.C. trip that takes place during their 8th grade year.
