Below you will find links to the downloadable forms. Feel free to print out any form that you need and drop them off at the office.
Medication and Medical Plans
Fill out the proper Medical Plan or if your child needs to take medication during school hours, the proper medical forms must be completed and returned to the school office. The completed form and the medication should be brought to the office by the parent. Do not send medications to school with your child. Medications need to be clearly marked in the original container or prescription bottle.
Change of Address
Notify the school if you have a change of address. The change of address form along with 2 proofs of residency need to be submitted to Student Services. Administration Building. Bus stops will be updated after the paperwork has been received.
Grades, attendance, and lunch accounts will be available through this link. Please fill out the form that is linked here, and have your student bring the completed form to the school office. You will then be assigned a PIN number and password through e-mail. Any question regarding Parent Connect, should be directed to firstname.lastname@example.org.
Permanent Bus Change Request
Permanent bus stop changes can be requested through the Transportation link on this website. Use this form if you require a permanent change for your child for the school year. Completed forms should be submitted to the school office for approval.
Internet Acceptable Use Policy
Before students are allowed to access the school's network, students and parents must first sign the internet policy use form. This gives students and parents the guidelines of what is expected of students when accessing the world wide web.
