Medication and Medical Plans
To obtain the medical forms, please go to Our District on the districts main page, then scroll down to Health Information. Here you may print and complete the forms to accompany the medication. Please note that all forms, prescription and over the counter do require a docor's signature.
Change of Address
Notify the school if you have a change of address. The change of address form along with 2 proofs of residency need to be submitted to Student Services. Administration Building. Bus stops will be updated after the paperwork has been received.

Permanent bus stop changes can be requested through the Transportation link on this website. Use this form if you require a permanent change for your child for the school year. Completed forms should be submitted to the school office for approval.

Internet Acceptable Use Policy
Before students are allowed to access the school's network, students and parents must first sign the internet policy use form. This gives students and parents the guidelines of what is expected of students when accessing the world wide web.
